Cremation Permit Request Process

The Montgomery County Coroner's Office has developed a new way for funeral homes and crematories to submit, safely pay by credit card, and receive cremation permit requests through our secure software. This will streamline the process, increase approval time, and do away with monthly billing. Credit card information will be required upon submission of the cremation request. Once approved, the charges will be applied, $50.00 for the cremation permit fee and a separate $1.33 credit card fee. The cremation permit fee must be paid at the time of submission.

Funeral home and crematory service representatives will need to create a new user, requiring an e-mail to log on. Please include your fax number on your account, as you will be able to choose how the approved cremation permit is sent to you: either by fax or e-mail. Once the account is created, you can submit cremation requests. First the decedent information will be input, then the death certificate submission will need to be selected from the drop down. The death certificate field has three options:

Death Certificate (DC) Issued by Coroner's Office- If the death certificate was issued by the Coroner's Office, you do not need to upload or fax the death certificate.

Upload Attachment- This is the preferred method if the Coroner’s Office did not issue the death certificate. Upload a digital copy of the completed death certificate or medical certification worksheet.

File Will Be Sent by Fax- Fax the completed death certificate to the Coroner's Office upon submission of the cremation request. The permit will not be approved until the DC is received.

After checking the acknowledgement box, you will be able to submit your cremation request. 

Next, you will be prompted to input your credit card information. 

The Coroner’s Office will receive your request. Once the request and death certificate are reviewed, investigated, and approved, the cremation approval form will be returned to you via your requested response method, either fax or e-mail, and the charge will be applied to your card. 

The implementation of this new system will allow users to submit cremation permit requests, check the status of pending requests, and view account history for previous permits.
