Victim Forms

On this page you will find two victim forms that can be completed on the computer and submitted electronically via email.

To do this, please download the form to your computer.  Complete the form and save it to your computer for your records.  You can do this by right mouse clicking on the link and select "Save Link As" or Save Target As".  Once the form(s) have been completed, please email it to VOJO@MontgomeryCountyPa.gov
