Connect with FREE Montgomery County Family Supports in all of Montgomery County!

Pregnant women and families with children under age six can benefit from evidence-based family support programs shown to improve maternal and child health, prevent child abuse and neglect, and improve family literacy and economic security. 

In 2015, the Maternal and Early Childhood Consortium (MECC) of Montgomery County initiated conversations to coordinate family support services in the county. In August 2018, five Home Visiting programs started a pilot Coordinated Referral System in Pottstown. Currently, there are six participating agencies with ten evidence-based programs:

  • Maternity Care Coalition’s Early Head Start and Healthy Families of America programs
  • Family Services’ Parents as Teachers
  • MCH Consortium for Chester County’s Parents as Teachers
  • Jefferson Einstein Montgomery Hospital’s Nurse-Family Partnership
  • Carson Valley’s Parents as Teachers
  • Montgomery County’s Office of Public Health Nurse Home Visiting Program

Our goal is to make it easy for you to connect with the family supports that you desire. Fill out one referral form and submit it via fax, phone or e-mail below, and we will connect you with the program that seems to be the best fit. If you have a program preference, please include it on the referral and we will connect you with the agency of your choice.
