Default
File a praecipe for judgment, using our form, a dated copy of the 10-day notice, and a Pennsylvania Rule 236 form. If the debtor is an individual, you must file a notarized affidavit of nonmilitary service. File an original and a copy for each debtor, with a Rule 236 form on top. Include a stamped envelope addressed to each debtor.
District Justice
File a certified copy of the judgment from the district justice for each debtor and a Rule 236 form. File the original and a copy with a Rule 236 form on top for each debtor. Include an addressed stamped envelope for each debtor.
Complaint & Confession
File the complaint in compliance with Rule 2952, verification, the confession of judgment, a copy of the judgment document showing the debtor's signature, and a Rule 236 form. File an original and a copy with a Rule 236 form on top for each debtor. Include an addressed, stamped envelope for each debtor. To file for money and possession, file the complaint and two confessions. The complaint must be approved by solicitor before numbering and time stamping.
Foreign / Out of State
File certified copies of the docket entry stating who the judgment is in favor of and who against with the money amount certified, certificate of addresses, Rule 236 form, and a notarized affidavit of validity. File the original. Include a complete copy of the judgment, with a Rule 236 form on top for each debtor. Include an addressed, stamped envelope for each debtor.
Arbitration Award
File a praecipe for judgment on the arbitration award 30 days after the award. Use a current prothonotary form and file the original and a copy with the Rule 236 form on top for each debtor. Include an addressed, stamped envelope for each debtor.
Arrearage in Support
File the praecipe for judgment, an affidavit of arrearage from domestic relations office, certification of addresses, and a Rule 236 form.File an original and a copy with a Rule 236 form on top for each debtor. Include an addressed, stamped envelope to the debtor.
Transfer to Another State or County
Request an exemplified copy of the docket entry with money amount certified. Include a self-addressed stamped envelope.
